- Navigate to Self Service Options on the left-hand side menu in Digital Banking. Then click Name Change Request.
- You will select the name record. Once selected, the ‘Start Name Change Request’ button will display. Upon clicking Start Name Change Request, you will be prompted to complete the request form.
- The Name Change form will automatically open for you to complete.
- The following fields will need to be populated and the form must be signed with a valid updated ID attached. Each field has helpful hints to what belongs in that field.
- New Name: First Name, Middle Name (optional), Last Name, Suffix (optional) – Enter your First Name as it appears on your ID
- ID Info: ID Type, Issuing State, Number, Issue Date, Expiration Date
- Signature
- Upload ID
- Upon clicking ‘FINISH’, you will be redirected to our website homepage. Closing the tab brings you back to where you left off in Digital Banking.
